The evaluation and analysis tools section includes downloadable instructions and tools for health care providers to analyze the effectiveness of implemented standardized communication tools.  These tools were utilized during pre and post intervention data collection and data analysis; where possible, further information can be found on the links provided.  Each section provides data collection tools as well as guidelines for collecting data in your work environment.   The two major sections of the evaluation and analysis tools are:
